The secretary of state requires that a person maintain one year of abstinence from alcohol and drugs before applying to get your license back.

A Request for a Hearing form will need to be completed prior to your evaluation at J & A (p. 1- 7). You can find the Request for a Hearing form on our website. Please fill it out accurately and completely as you will need to send it in with your packet to the Secretary of State. Dates included in the request for a hearing form must be as accurate as possible and not left blank.

You must go to the Secretary of State and request a copy of your Master Driving Record and submit it with your paperwork to our office. 

You will need to provide 3 to 6 letters of support from people that can verify your length of abstinence from alcohol and drugs and the lifestyle changes that you have made. You can find a list information needed for letter writers on our website under Community Support Letters. The support letters that you receive from people will need to be notarized before you send them to the Secretary of State. 

You can submit any other documentation that supports your abstinence from alcohol and drugs such as: program completion certificates, support group meeting attendance, probation/parole discharges etc. 

If you are taking prescribed medications for pain or a mental health condition you will need your doctor to fill out a Physicians Statement required by the Secretary of State. You can download the Physician’s Statement from our website. *If you are taking multiple medications, please get a list of the medications from your pharmacy. 

If you have a medical marijuana card, you will need to show proof of it and you will need to document why you are using medical marijuana and provide the doctors name who prescribed it for you. 

If you have a breathalyzer installed in your vehicle, please obtain a copy of the most recent BAIID report from your interlock provider and include it with your paperwork. It would also be very helpful if you provided the evaluation that was completed with you to get your restricted license for consistency purposes. If the evaluation was completed at J & A Counseling, we have a copy of your initial secretary of state evaluation. 

If you are being referred by an attorney, please have their office email all of your paperwork to hsbartlett@jacounseling.com prior to scheduling an appointment. 

Once you have all of your paperwork completed you can bring it to our office Monday-Thursday in between 9am-3pm. At this time, you will provide a urine screen and make an appointment to do the Substance Abuse Evaluation required by the Secretary of State. 

If you do not plan to complete a urine screen at our office, please provide a copy of your Secretary of State compliant urine screen with the rest of your requested paperwork. 

Payment for the evaluation is required at this time. If you are doing a urine screen at another facility, please provide a copy of the results with your paperwork. If you have questions after reading this, please call our office at 989-631-4092.

A urine screen will be included as a complimentary cost of the Substance Abuse Evaluation at J & A Counseling. 

If you chose to do your urine screen at another facility the cost of the Substance Abuse Evaluation will remain the same. 

If you test positive for drugs that you do not have a prescription for, we will be unable to complete the evaluation and you will not get a refund for your evaluation. 

If you test positive for alcohol you will not get a refund for your evaluation. 

If your urine sample is diluted, you will have to submit another urine screen as The Secretary of State does not accept dilute results. The cost of the retest is $65.00 

All positive drug and alcohol tests must accompany any evaluation that is completed by J & A Counseling.
